What you need to do:
- Visit our reception desk for registration of your personal details (A form will be given to fill in) or download New Patient Registration Form, so we can pass this information onto your doctors.
- Need to sign an authorization form to transfer your medical record from your previous doctors.
- Make a longer appointment with your doctor.
- Bring all your necessary documents and medication list at your first appointment with your doctor..
